Male factors are likely to be present in 50% of infertile couples, with primary hypogonadism the most common reason. Imaging is a crucial aspect in the patient assessment, with ultrasound the main investigation, with magnetic resonance imaging a useful adjunct in problematic cases. Testicular texture, volume and appearances of the epididymis indicate a potential cause for the infertility. Varicoceles are normally prominent, and may be assessed dynamically using ultrasound. Tumors are readily detected, epididymal obstruction can be seen. Imaging of the prostate and the seminal vesicles may be obtained with ultrasound, using a transrectal approach, supplemented with magnetic resonance imaging for better delineation of the ejaculatory pathway. Imaging pathways are essential in the workup of the male infertile patient, with skills of the operator paramount to correct diagnosis.
