ABSTRACT
The importance of post-accident investigation is widely recognized in industrial sectors involving the threat of major accident hazards, as defined in the European Directive 96/82/CE ‘‘Seveso II’’ (Seveso II, 1997): in fact, by examining the dynamics and potential causes of accidents, lessons can be learned which could be used to identify technical and managerial improvements which need to be implemented in an industrial context. This is in order to prevent the occurrence of accidents and/or mitigate their consequences.
