ABSTRACT

This chapter deals with reflections on the applicability of the triangle model, as well as how its embodiment may be influenced by health system characteristics. It brings the triangle model to life in the context of a demonstration project to improve colon cancer services for one million residents in southeastern Sweden that involves “healthcare professionals, patients and their families, researchers, payers, [leaders,] planners and educators,” spanning multiple organizational boundaries. With inspiration from a collaborative improvement effort targeting colon surgery more broadly, clinical leaders in surgery, oncology, and palliative care have articulated a regional best practice guide for colon cancer. In parallel with the colon cancer demonstration project, the regional health-care leadership established its Regional Cancer Center based in Linkoping. The efforts to improve colon cancer services in southeastern Sweden – from prevention, diagnosis, and therapy to palliative care – exemplify a number of ways that the triangle model.
