ABSTRACT

This study explored the use of Line Operations Safety Assessment (LOSA) (also known as Line Operations Safety Audit) incorporating the Threat Error Management (TEM) model and flight data management technologies to determine the prevalence and quantity of threats, errors and undesired aircraft states (UAS) that occur during a normal flight in a collegiate aviation flight training program. Digital image, voice and flight data recorders were installed in two PA-28-201 Piper Arrow aircraft used for university FAR Part 141 flight training.
