For each of the following people with thyroid disease, select the most likely diagnosis. Each option may be used once, more than once or not at all.

1. A 50-year-old woman attends her GP complaining of low mood and lethargy. She has had this for a few months and has had much time off work. On further questioning, she admits to being constipated and to having gained weight. On examination, no abnormality is apparent in the neck and no lymphadenopathy is palpable.
