For each of the following scenarios, select the most appropriate diagnosis. Each option may be used once, more than once or not at all.

1. A 56-year-old man presents to the GP with a 2-month history of excessive urine production, constipation and general lethargy. A set of blood tests is requested, and shows a corrected calcium of 2.8 mmol/L (reference range 2.2-2.6 mmol/L) and a significantly raised parathyroid hormone.
