1. A 56-year-old man with chronic renal failure has become gradually more lethargic over the last 2 months. On examination, he has pale palmar creases and pale conjunctivae. Auscultation reveals an ejection systolic murmur. His full blood count shows a haemoglobin concentration of 8.2 g/dL and a mean cell volume of 85 fL. The ferritin level is normal.
