Once the patient is in the anaesthetic room, these checks are repeated partially or completely. In addition, in the UK, the NPSA has recommended a preoperative verification checklist, which includes details of the patient, the proposed operation, and the site and side of surgery. There are four checks; and these are completed by the operating surgeon and the ward staff and also involve the anaesthetist and theatre team. The final check is performed when the patient is in theatre, and the surgeon asks the anaesthetist and theatre team to confirm verbally that the correct patient is having the planned procedure on the intended site or side.
