ABSTRACT
A 34-year-old woman presented with a 3-hour history of palpitations. She is mildly dyspnoeic and has some chest pain. She has been feeling anxious and suffering from insomnia for the last few weeks. On direct questioning she admits to a 10-day history of diarrhoea. On examination she has a tachycardia and her pulse is irregular.
