ABSTRACT

This paper describes the results of a project to develop a methodology to evaluate the Probability of Failure on Demand (PFD) for human operators responding to alarms in process plants. The project investigated the use of expert judgement based techniques as a source of Human Error Probabilities that could be used to evaluate PFDs. The paper presents a case study from the oil and gas oil and gas sector of the use of the Success Likelihood Index and Influence Diagram techniques to generate human error probabilities. The possibilities for collecting human error data via the use of training simulators and the process control systems found in the oil and gas sector will also be discussed.
