The malfunctions or failures of computer system components, incorrect documentation, data errors, improper operation, or interface errors illustrate some of the incidents that can affect the correct operation of a computer system. These system incidents are also nonconformances.*

Effective monitoring of the operation of a computer system also involves user or operator training in proper operation. This facilitates their ability to recognize unexpected responses and outputs and to fully document such incidents to aid in the evaluation and debugging process.
