ABSTRACT

The most readily accessible source of laser lessons learned is from the department of energy laser safety task group (DOE–EFCOG) Laser Safety Working Group. Laser incidents are of great concern to the US DOE. Each incident gets an investigation with a written report. Information from those reports in a summary format can be found of the website of the DOE EFCOG laser safety working group. In addition to the lessons learned on this website, one can access the laser safety officer workshop presentations. The workshop serves as the official annual meeting of the US DOE EFCOG. The MAUDE database is freely accessible to anyone. One can go to the food and drug administration or CDRH website and search for databases to find the current link to this resource. The laser accident database is maintained by Rockwell laser industries. As with any database, it is only as good as its data.
