Pulmonary-only lesions For pulmonary-only lesions, all previous studies have suggested a benefit of pulmonary resection6-11 (Table 12.4). Median survival is approximately 2 years, and 20-30% of patients can be expected to survive for 3 years. It is important to emphasize, however, that not all patients with apparent pulmonary-only metastasis are resectable. In a review from MSKCC12 of a total of 716 patients with primary extremity soft tissue sarcoma, 135, or 19%, developed isolated pulmonary metastasis. Of these, 23 (17%) had locally recurrent resectable disease, while 112 (83%) had isolated pulmonary metastatic disease. Fourteen of the 112 previously had a local recurrence prior to the development of pulmonary metastasis, that is, 31% presented with synchronous pulmonary metastasis and 69% metachronous.
