ABSTRACT

Tissue-sparing repair (TSR) has been used since 1988 in over 3000 surgeries to decrease wound length. Fusiform surgical excision uses a 3.5 to 1.0 excision to lesion ratio, whereas TSR decreases wound length by taking a surgical circle and making it a line. The TSR ratio is 1.5 to 1.0. This requires appropriate planning and the use of the horizontal oblique dermal suture to approximate tissue and the apex cutaneous suture to remove dog ears (1). The technique is simple to learn.
