THE PARAMEDIAN FOREHEAD PEDICLE FLAP FOR NASAL RECONSTRUCTION In the 27th edition of Dorland’s Illustrated Medical Dictionary, a pedicle flap is defined as ‘‘a flap consisting of full thickness of skin and the subcutaneous tissue attached by tissue through which it achieves its blood supply.’’ In practice, most pedicle flaps remain attached to their source for a period of several days to a few weeks. Viability is thus assured by establishment of a blood supply in the recipient bed. In dermatologic surgery, the three most common pedicle flaps utilized are the paramedian forehead flaps for nasal reconstruction, the superiorly based naso-labial pedicle flap for extensive defects of the ear. This chapter will present applications of the forehead pedicle flap for nasal reconstruction. (RKR also commonly prefers the subcutaneous island pedicle flap, especially for upper lip defects, as discussed in Chapter 25).
