Evaluation and Diagnosis Clinically, bed partners may report kicking, leg jerking, or twisted bed sheets and bed covers in the morning, but most patients themselves are unaware that PLMs are occurring. As a result, PLMD remains a polysomnographic diagnosis. They can also be reliably diagnosed by actigraphy, using accelerometers attached to the legs (16,17). This has the advantage of portability and lower cost, but does not collect any other information about sleep. Currently, actigraphy is usually reserved for investigational trials.
