ABSTRACT
CHALLENGING CASE A 26-year-old man has a 2 month history of perianal itching. He has variable bowel movements and no family history of colorectal cancer. Physical exam demonstrates thickened perianal skin with ridges in a circum anal pattern. The sphincter tone is normal and no masses or tenderness is appreciated.
