Two randomized studies comparing carotid angioplasty and CEA30−31 showed comparable results. Restenosis is fortunately rare after CAS, between 3 and 5%, and the long-term results are encouraging.22−28

So CAS may be a substitute for surgery and may be proposed to the majority of patients suffering from a carotid stenosis, in particular high-risk patients. However, embolic stroke, even with a meticulous technique and experienced operators, represents the major drawback of the procedure. The majority of neurological complications are due to intracerebral embolism of plaque fragments or thrombus during different procedural steps.
