ABSTRACT
Medical errors in the operating room (OR) environment can cause significant morbidity and mortality. The OR can be a high-pressure environment with time constraints affecting performance even further. It is a complex multifaceted system with built-in capacity for system failures in addition to individual provider surgical error. This article is intended to review a system-based approach to the reduction of errors encountered in the OR environment.
