NORMAL CORONARY ANATOMY AND CORONARY ANOMALIES Normal coronary artery anatomy is characterized by two ostia centrally located in the right and left sinus of Valsalva of the aortic root, slightly cranial to the aortic valve. The main left coronary artery originates from the left ostium, branching into the left anterior descending coronary artery and circumflex coronary artery. The latter follows a course in the left atrioventricular groove, while the former travels in the anterior interventricular sulcus toward the apex of the heart. The right coronary artery arises from the right coronary ostium. The vessel then follows the right atrioventricular groove and usually ends as a posterior descending artery in the posterior interventricular sulcus (Fig. 19.1).
