What Makes a Lesion “Complex” in Iliac Interventions? Although over 90% of iliac lesions can be passed with simple guidewire techniques, each endovascular attempt to treat an iliac occlusion might be compromised by failure of guidewire passage. In order to achieve optimal success when treating complex iliac lesions, the operator should know when and how ipsilateral, contralateral, and brachial access techniques should be applied. Later in section “Interventional Strategies,” we will discuss the different interventional strategies that can help to treat a complex iliac case successfully.
