Collection and Drainage of Cord Blood At CD, drainage of fetal blood from the umbilical cord is associated with less incidence of feto-maternal transfusion (measured by Kleihauer-Betke test) compared with no drainage (54). The clinical significance of this finding is unknown.

Placental Removal Placental removal options are either spontaneous (with gentle cord traction) or manual (55). Manual removal of the placenta is associated with more endometritis (RR 1.64, 95% CI 1.421.90); more blood loss (by 94 mL); more blood loss >1000 mL (RR 1.81, 95% CI 1.44-2.28); lower hematocrit after delivery (by about 1.6%); greater hematocrit fall after delivery (by about 0.4%); and longer duration of hospital stay (by about 0.4 days). The duration of surgery was shorter in one trial but not overall.
