ABSTRACT
The extra-contractual referral (ECR) was an administrative and financial device whereby doctors could refer and health authorities could fund patients to be treated at institutions other than those providing the majority of care for a given authority. The need for this device resulted from the creation of the NHS ‘internal market’ in the Conservative Government White Paper, Working for Patients, published in 1989.
