ABSTRACT
This chapter presents some closing thoughts covered in the preceding chapters of this book. The book provides a practical and useful guide to health care professionals, specifically nurses, both pre-registration and those embarking on practice nursing/community nursing careers on the wide ranging aspects of implementing and providing care packages and promoting self-management advice for people with long-term conditions (LTCs). The book includes an update on policy, not everyone's favourite topic, but a necessary and important one to inform the reader of what's happening regarding support, care and treatment of people with LTCs. Increasing elderly population LTCs are clearly set to increase and this continues to be identified and addressed by health and policy leaders worldwide. The book is about sharing knowledge and written by utilising the excellent sources of information included in the extensive reference list.
