This was the last manuscript Dr. William Freudenburg and I collaborated on prior to his passing on December 28, 2010. “Bill,” as friends and colleagues called him, was unable to complete the requested manuscript revisions with me; consequently, I have made revisions to the original piece without the benefi t of his scholarly perspectives and interpretations. In most cases, such revisions are entirely consistent with and integrated into our original themes, but in a few places I have inserted blocks of italicized text that represent “editorial commentary” and views that Bill may or may not have fully shared. Hopefully, the reader will not fi nd these brief passages to be disruptive, as it has been my strong intent to leave Bill’s voice and essential message of this manuscript intact.
