Scholars nevertheless intuitively suggest that there is an organic link, somewhat still obscure, between the division of labour, the organisation of activities and the safety and reliability of an organisation (Ackermann and Bastard, 1991; Clarke, 1989). Moreover, organisational failures such as Chernobyl (Reason, 1987, 1990b), the French blood scandal (Setbon, 1993), or the Challenger accident (Vaughan, 1996) have indeed emphasised the contribution of organisational factors.
