Human factors as revealed by the use of natural language in near-incidents at nuclear power plants

Analysis of near-incidents likely to be caused by human error has led to numerous important suggestions for ways to prevent human error (Labour Safety Institute, 1994). Nevertheless, near-incidents reported in recent years have shown a trend involving four problems in particular:
