ABSTRACT

The care that a patient receives is seldom provided by a single organisation. For many clinical pictures and in cases of chronic illness in particular, the patient comes into contact with multiple care organisations. The quality of the care and the outcomes of the care are thus the sum of the efforts of many different parties. It is rarely the case that the outcome of an intervention depends upon just one process from a single organisation. The improvement of treatment and care outcomes must therefore occur through dialogue with other organisations.
