ABSTRACT
In the Review, we always try to keep the description and explanation of events separate from assessment and policy recommendations. I shall do the same here. The sections that follow are intended as a detached commentary on the figures in the tables that go with them. What might be called the ‘editorial matter’ (which would be called an ‘appraisal’ in the Review), comes separately at the end of the chapter.
