The next major change was after one year of once-weekly therapy. I was suddenly aware that when Steven banged his head he was making enormous spitting noises and sound effects but in fact he was miming banging his head. When I said he was not hurting his thoughts so much and he was thinking more about what I said, he lifted his hand to show me the bumps had subsided. I told the home staff about my observation and they then realised it was true in their environment too.
