ABSTRACT

Diabetes mellitus is a growing, complex, and costly chronic disease in the United States. The disease is increasingly prevalent, is frequently associated with comorbidities, and requires multiple medications and intensive behavior change in its management. Evidence-based standards of diabetes care exist, which emphasize coordinated medical care and patient self-management. Coordinated care management includes identifying patient health care needs, customizing medical care and patient self-management support, and coordinating ongoing care across providers and settings and with patients to achieve long-term, optimal control.
