ABSTRACT
This chapter must be prefaced by what amounts to an academic health warning. In the first place, it was written in 1990 as a report on work in progress. It was apparent to me then that there were still many loose ends, open questions, unfollowed leads and unseen documents. Little has changed in the intervening years, save that I no longer have access to many of the files on which this article is based.
