ABSTRACT

Patients and families spend more time living with their conditions than people do. Patient engagement remains our biggest challenge if people are going to truly affect the health of the populations people serve. Emergency visits often occur because patients and families do not have access to the right person in the right place at the right time. Patients who understand their conditions and have confidence in their self-management skills have lower health system usage and better patient outcomes. Behavioral choices determine approximately 40% of our health, and poor choices by patients generate huge costs today and into the foreseeable future. The team decided to change the learning model first. Patients, families, and educators reported that repeated lectures of didactic content were boring. The clinical team determined the learning benchmarks needed to successfully manage diabetes. The video session was purposefully designed to encourage patients and families to address those themes.
