ABSTRACT

A state-wide health information exchange (HIE) received grant funding to participate in a national pilot for better coordination of care between hospitals and primary care providers (PCP). The HIE would work with its main technology vendor to develop a system to alert a PCP when an affiliated patient was admitted to and discharged from any hospital participating in the HIE. The HIE received a data feed of all hospital and emergency department admissions and discharges from participating hospitals, which would provide the basis for communications to PCPs. The HIE began to introduce the “Provider Notification” solution to PCPs across the state using the state’s Regional Extension Center staff who were already assisting the same PCPs with EHR adoption. Several PCPs, including a 100+ physician Medicare Pioneer Accountable Care Organization, agreed to participate and pilot the solution.
