ABSTRACT

Comprehensive review of family charts has always been a central function of the Health Care Evaluation Committee. Initially, family chart review was performed weekly by members of the committee on one family chart pulled at random. Audited teams were rotated weekly to assure that each team was audited with the same frequency. The issues of audit involved the presence and maintenance of a problem list and gross errors of medical judgment. Numerous difficulties quickly became apparent. Family folders for review had to contain at least one adult's chart and one child's chart, but no more than a total of six individual charts. The new form for family audit consists of three sections: Family Audit Form, Family Audit Form and Pediatric Chart Review. The reviewing team must have an internist, pediatrician, nurse practitioner, and family health worker involved in the review.
