ABSTRACT
Major failures in large-scale complex systems can provide useful sources of feedback about faulty practices and procedures, but the volume of information generated by major inquiries often inhibits optimal learning. A simple visual technique for summarising information relating to such malfunctions is described and illustrated. The Schematic Report Analysis Diagram may also be used to link evidence with recommendations for change, and to disseminate more widely the findings of a failure investigation.
