ABSTRACT

This chapter describes the findings of the pilot scheme and the conclusions drawn about the efficacy of this approach that have led to its further local development. It also describes the evaluation of a pilot project of Community Assessment and Rehabilitation Teams in Cornwall. The radical reforms that have occurred in the health and social services during the early 1990s have many implications for the management of elderly and disabled people in the community. In addition to the statutory requirements upon the health and social agencies for assessment, there is the important question of the rights of the individual to receive appropriate care. The medical assessment led to frequent referral to therapy departments, chiropody and other modalities of care. Hospitalisation is often the final factor precipitating permanent entry to care. Although a minimal length of stay in hospital is clearly desirable, it should not be at the expense of re-admission.
