ABSTRACT

A personal account of organizing care around a vulnerable person living at home is not going to be fundamentally different from what others are doing, as they do what they can in a wide range of circumstances. This chapter explores whether it is really possible to plan for an integrative way of working that would meet the needs of vulnerable people for continuity and consistency in their care. The negotiating partnership model provides the conditions for the effective use of resources according to need, with continuing review of good practice, while providing continuity and consistency of care and reducing social isolation. The partnership model would make little distinction between the formal and informal care being provided. A process of negotiating partnership should allow for the development of appropriate boundaries.
