ABSTRACT
We want to describe here a model of work established over the last six years in several GP practices in West London. We explain the context, give a historical overview, and outline the theoretical framework that guides the work as well the nature of referrals, giving some case examples. We provide data on the evaluation of the service, explain how this model provides continuing professional development for the multidisciplinary team in primary care, and consider the future development of the service.
