ABSTRACT
We are delighted to present to you, the reader, with the wealth of knowledge of a wide range of professionals working in primary care. They have two things in common – a deep understanding of the power of primary care to improve the quality of people’s lives and a passion for working together through collaboration – at its heart this book is about what that means and how to bring effective collaboration to life. Exactly who those healthcare professionals (HCPs) are will be described more fully through the course of the book, although for the moment we will mention doctors, nurses, pharmacists, paramedics and social work practitioners. Others will follow.
