ABSTRACT

Everyone experiences transition in their lives. The transition from the hospital to the community for clients who experience challenges with mental health depends on the ability of healthcare providers to develop therapeutic relationships and sustain specific conditions for success; however, these elements require support from quality work environments and strong leadership. The transitional discharge model is one intervention that uses the provision of therapeutic relationships to support clients’ transition from the hospital to the community. This chapter explores practical considerations for integrating the transitional discharge model in the clinical setting. Topics of discussion include work environment and leadership; practice, policy, procedures, and protocols; staffing and scheduling; work assignments and workload; communication; documentation; education; and technology.
