ABSTRACT

This chapter presents an account of the author's experience as a health and safety manager. He talks about his discussion with a health and safety auditor to provide an engineer with new climbing Personal Protective Equipment with an accredited inspection certification. The lesson learned from the author's experience is that the health and safety practitioners must be able to change their ‘standard’ safety investigation methodology and trajectory as they uncover evidence and facts. Something might initially seem straightforward to conclude; however, a different investigative methodology may pull the layers away, reveal complex underlying issues that harbour the true cause(s) and produce completely different outcomes.
