ABSTRACT

An important topic for therapists to understand is the complex dynamics of the interdisciplinary care team, inclusive of the patient and the family. Historically, therapists are not consistently an integrated part of the palliative care interdisciplinary team (IDT). Yet, in order to best advocate for individuals with a terminal illness and their families to achieve their goals, the therapist must understand the roles and dynamics of the myriad professionals across the continuum of care. This chapter will outline the common roles and impact of commonly encountered palliative care team members and best practices in order to facilitate therapists’ integration into the care team in an efficient and effective manner. In addition, commonly encountered challenges and opportunities will be described, including establishing effective and professional communication, appropriate and situational communication strategies, and considering the “chain of command” within the palliative care IDT.
