ABSTRACT
SSFC is a model of brief family engagement and inclusion in the care of an individual client. Originated at The Bouverie Centre, the development of SSFC is described. The practices of C onvening, Conducting, and Following-Up an SSFC are illustrated with an unfolding case example.
