ABSTRACT

Digital health is a global trend where digital technologies are adopted to transform health care services. New application of technologies, such as smartphones and social media, is transforming the way that we are tracking our health and giving access to new channels of health information. This includes, but is not limited to, digital solutions involving telemedicine and telehealth, mobile health (mHealth), mobile applications (apps), wearable devices, remote monitoring and the traditional electronic health records. Digital health also encompasses a broad scope of new techniques that engage patients for clinical purposes, including new forms of behavioural and health data, as well as interdisciplinary collaboration for new data analytical approaches, such as machine learning on pattern recognition, data interpretation for clinical diagnosis and management or other measures of health care quality. In the long run, innovation in digital health will create an ecosystem where health care is accessible, cost-effective and more efficient for population health. This chapter will introduce how digital interventions could be used with community engagement to promote chronic disease management.
