ABSTRACT

This chapter highlights the concurrent national dissemination of evidence-based chronic disease self-management education (CDSME) and fall prevention programs in rural communities from January 2009 to January 2019. Many credible organizations have developed their own criteria for defining evidence-based programs (EBPs) for older adults. One such organization is the Administration for Community Living (ACL), a division of the United States Department of Health and Human Services. The primary purpose of the ACL funding initiatives is to develop capacity for, bring to scale, and/or sustain evidence-based CDSME and fall prevention programs. EBP topics include chronic disease self-management, fall prevention, physical activity, caregiving, behavioral health/mental health, and general wellness. While a larger proportion of CDSME participants from urban areas participated in residential facilities than in health-care organizations, a larger proportion of CDSME participants from rural areas participated in health-care organizations than in residential facility. Health care was the most predominant in rural counties for CDSME.
