ABSTRACT

Social workers have been privately providing geriatric care management since at least the 1980s, when they met with nurses and psychologists to discuss their cases in New York City. In this chapter, readers will learn about Joan, a white, heterosexual, single woman in her seventies who urgently needs support due to multiple chronic health conditions. The licensed social worker and geriatric care manager role is delineated in the context of helping Joan navigate the challenges of aging in place. The extensive work of mediating among the court-appointed guardian, the care facility, and the client is described and critiqued.
