Do I deserve to be a clinician? Am I making things better? We all have crises of confidence. A patient, Mair, is described, after she suffered a right middle cerebral artery stroke. Her rehabilitation seemed ‘stuck’ for some reasons. On assessment, difficulties in memory and learning are identified, and the treatment programme is adapted. This appears to be working, but then fails, with Mair increasingly distressed. This prompts a reflection on the formulation, and a change in approach to the management of the case.
