ABSTRACT

As seventy-five to eighty percent of accidents continue to be attributed to 'the human factor' the blunt implication of that statistic would seem clear: the design, manufacture and operating procedures of an aircraft are unlikely to be the root cause of an accident whereas failure in the operator's decision making is. The focus in human factor's training is not to teach the trainee to recognise and respond to visual prompts, instead the focus is on why we so often chose the wrong response, no matter how well learned, and to identify under what circumstances we are most vulnerable and likely to do so. LOFT was designed and developed to provide a non-critical yet realistic environment where crews can observe their own behaviours and see the implications of their actions, as the events unfold. The Human Factors Proficiencies serve as both a training record and/or a list of guidelines depending on the requirement.
