ABSTRACT

According to Kindig, when we talk about population health we are talking about “the health outcomes of a group of individuals, including the distribution of such outcomes within the group.” 2 Within the context of public health, groups of individuals have often been taken to be those in large population centers (> 500,000 people). Whereas there has been a more recent focus on population health issues at the zip code level, this effort has rarely focused on smaller population centers, or rural communities.
